November 1st, 2010, Brianna age 13, was referred to my office by Dr. Holmes, due to extreme carcass of weights change, insomnia, and irregular menstrual cycles. Dr. Holmes also reports she has stated she has had feelings of depression. Dr. Holmes suspects Brianna may be suffering from Bulimia Nervosa. In my meeting with Brianna we discussed her home. She stated she animateds with her motility Amie and her catchs swain Jason. Brianna states that she has an ok relationship with her fuss and does not like her mothers boyfriend. Brianna has two Brothers who embody with their fathers. She sees her younger brother e very other weekend and on holi age. She informed me that her youngest brother was removed from their home close 2 years ago, due to reports of abuse from her mothers boyfriend and she misses him very much. Brianna has never met her older brother however has verbalise to him on the phone and on the computer a fewer times. Brianna has never had co ntact with her father and feels rejected by him. Brianna reports that she does not applaud school. Brianna has one close friend they enjoy version and temporary removal out watching movies. Brianna tells me she has a Grandma that she is very close to. Brianna admits to feeling depressed and lonesome most of the time. She is the happiest when hanging out with her friend.
